Kaitlyn Forman, LCSW – Glenwood Springs Integrated Health Center
Katie Forman, LCSW received her Master of Social Work degree from Smith College School for Social Work in 2015. She moved to the Roaring Fork Valley and began working for Mountain Family in 2021 as a behavioral health provider. Prior to moving here, she lived in Lakewood, Colorado and worked as a therapist for CU Boulder and Denver Health. Katie has experience working with adolescents and adults; has received specialized training in substance abuse and trauma, and takes an individualized, integrative approach to therapy, combining psychodynamic (relational, talk therapy) with skills-focused modalities (Dialectical Behavioral Therapy, Cognitive Behavioral Therapy, and mindfulness). Katie feels grateful to be close to water again, for the abundance of outdoor opportunities, and to have the general privilege of getting to now call the Valley home.
Adriene Fedde, MD – Basalt Integrated Health Center
Dr. Adriene Fedde joined Mountain Family in 2022. She was raised on a farm in southeastern Colorado and received her undergraduate degree from the University of Colorado. Adriene served as a Peace Corps volunteer in Niger prior to entering medical school. She attended the Medical School for International Health at Ben Gurion University in Israel. In 2008, Dr. Fedde completed a residency focused on rural, full spectrum family medicine in Santa Fe, New Mexico. She is excited about providing care to people of all ages and has interests in international, child, and women’s health. She enjoys traveling and spending time in the great outdoors with her family of four.
McKenzie Rieder, CPNP-PC – School-Based Health Centers
McKenzie grew up on the front range and lived in several different places, including Boston and Nicaragua, before deciding to move back to Colorado with her family. She earned her Master of Science in nursing from Boston College in 2018 and has been working in pediatrics ever since. With a background in quality improvement within community health, McKenzie is particularly passionate about ensuring access to quality healthcare across all demographics, but especially in the younger population. McKenzie is bilingual in English and Spanish. When she does have free time, McKenzie participates in her greatest pastimes: hiking, gardening, camping, and whitewater rafting with her husband, three kids, and their dog, Bruce.
Lindsay Williams, RDH – Basalt Integrated Health Center
Lindsay Williams grew up in Montrose Colorado and earned her Associate of Applied Science in Dental Hygiene at Colorado Northwestern Community College in Rangely in May 2022. She is compassionate, kind, and always willing to help her patients. Her goal is to improve oral and overall health, be a resource to her patients, and provide quality care. During her free time Lindsay enjoys spending time outside with her dog, horseback riding, and gardening.
