Insurance Programs

In May 2023, the U.S. Department of Health and Human Services declared the end of the Public Health Emergency (PHE) which provided continuous coverage for many Coloradans. All current Medicaid/CHP+ enrollees should update their contact information to ensure they receive proper notice when it is time to begin their renewal process. To avoid gaps in coverage, members need complete renewal packets and provide required/requested verifications before the deadline given on notices. As a Certified Application Assistance Site (CAAS), we are here to help facilitate this process.

In 2021, Colorado passed a new law that expanded the access to health insurance accessibility and coverage, regardless of citizenship status. More information about eligibility requirements can be found HERE.

Get insurance through Connect for Health Colorado

To ensure that Coloradans have the health coverage they need, the next Open Enrollment period through Connect for Health Colorado begins Nov. 1, 2023 – Jan 15, 2024. Special enrollment periods are still available to those with life changing events (did you lose coverage, get married, have a baby, etc.?)

The Connect for Health Colorado program has Health Coverage Guides and a website to help you find and purchase public or private insurance. You can apply for coverage, compare plans, and enroll. If you need assistance, you can also contact our Enrollment Specialists at 833-273-6627.

Sliding Fee Discount Program

The sliding fee discount program is available for those not eligible for Health First Colorado (Colorado’s Medicaid) or CHP+. Nominal fees range between $20.00 - $85.00, while lab fees range from $20.00 - $70.00. You may qualify based on the household size and income guidelines below. This program is not health insurance and is only valid at Mountain Family Health Centers.

Can’t make it to an in-person appointment with Enrollment? Request to be screened for financial assistance programs by following the steps below:

Step 1. Review the “Documentation Checklist” above for a list of requirements.

Step 2. Scan required documents with your smartphone or computer.

Step 3. Rename documentation appropriately for a clear understanding of the document.

Step 4. Click HERE to sign in (this will require first name, last name, and email; no company name needed)

  1. Load all the required documentation.
  2. Once you see each document on your screen, click upload in the bottom left corner.

The Enrollment Department will then review your documentation and follow up (within 3-7 business days) via email or phone call to inform you of programs you may be eligible for. Please note you may be asked to complete the enrollment process virtually, provide more information, or get scheduled for an in-person interview.

Health First Colorado (Colorado’s Medicaid)/CHP+

Medicaid, now known as Health First Colorado, was expanded as part of the Affordable Care Act. You and your family may qualify for Health First Colorado (Colorado’s Medicaid) or Child Health Plan Plus (CHP+) health care coverage. We can help you understand what is available, and we can help you apply.
