Mountain Family’s Outreach & Enrollment Department assists with completing financial assistance programs applications both virtually and in-person. These programs are designed to make high‑quality, whole‑person care more affordable for individuals and families in our community. These programs help reduce the cost of medical, dental, and behavioral health services based on household size and income. Whether you’re uninsured, underinsured, or facing financial hardship, we offer sliding‑fee discounts and support in exploring coverage options so you can access care with confidence and clarity.

Sliding Fee Scale Discount Program

The SFS Discount Program is available for any individual and/or family, including those who have private insurance. To apply, we have a list of required documentation which you may provide by scheduling an in-person appointment or by uploading HERE to complete virtually.

Medicaid/CHP+

Federally and state funded insurance programs that are administered by the Department of Health Care Policy & Financing (HCPF)

Medicaid: A public free, or low-cost, health insurance program for qualifying individuals and families. In July 2022 Colorado expanded income limits in hopes of enrolling more Coloradan’s, regardless of immigration status, into Family Planning Benefits. As of January 2025, qualifying undocumented children and pregnant women may be eligible for full Medicaid benefits. Click HERE to learn more!

CHP+: A public and low-cost health insurance program for children and pregnant women who are over income for Medicaid but do not earn enough to pay for private health insurance. Click HERE to learn more!

Connect for Health Colorado

Connect for Health Colorado is the state’s official health insurance marketplace, where uninsured individuals and families can explore coverage options and receive financial assistance to help lower the cost of private insurance. To qualify, you must reside in Colorado, plan to file a federal tax return, and be lawfully present in the United States. You may ask yourself, when can I buy health insurance? The answer is: you must wait until the Open Enrollment Period which runs from November 1st through December 15th. You may also shop outside of Open Enrollment Period if you have a qualifying life changing event, which opens a Special Enrollment Period for you and your tax household. Click HERE to learn more!
