Mountain Family Health Centers and Roaring Fork Schools have partnered to provide school-based health care in Basalt, Carbondale, and Glenwood Springs, in addition to our school-based center in Avon. When your child or adolescent is enrolled in one of these schools, you will have access to health care on campus, during or after school. Our innovative program will save you time and ensure access to care when and where your student needs it.

Integrated medical, behavioral, and dental health care will be provided in Basalt Elementary, Middle, and High School and Roaring Fork High School in Carbondale. Mountain Family will also open a school-based health center in Glenwood Springs High School this fall, in addition to our school-based health center in Avon.

WHEN THE SCHOOL YEAR BEGINS, WE WILL BE HERE FOR YOU.

School-based health centers are an affordable, convenient option for ensuring children and adolescents have access to the health and wellness care they need to be healthy and successful in school. Students can go to the health center to receive their care at their school during and after school, the place where they spend most of their time.  

Our Outreach and Enrollment Team can help parents and students apply for private or public health insurance to cover the cost of care. Parents who may have lost their employment or health benefits due to the COVID-19 pandemic, may now be eligible for Medicaid, federal subsidies for commercial health insurance coverage, or other financial assistance programs. Additionally, Mountain Family offers a membership program to families called Health Solutions. This program, which is not insurance, provides access to high quality, affordable integrated health care for a set monthly fee.

If your child requires more advanced care, or for care of other family members, Mountain Family also provides telehealth and safe, in-clinic, medical, behavioral, and dental care in Basalt, Glenwood Springs, Edwards, and Rifle. We also provide community testing for COVID-19 at our clinics in Glenwood Springs, Basalt, and Rifle. In addition, we offer antibody testing for COVID-19 at all locations when you schedule an office visit.

A full range of services is provided to students through the Mountain Family School-Based Health Centers:

  • Well child/adolescent visits
  • Acute illness/injury care
  • Chronic conditions management 
  • Behavioral health services
  • Substance abuse prevention and counseling
  • Sexual health education, testing, and treatment of sexually transmitted disease
  • Birth control counseling and initiation and continuation of all methods including hormonal and long acting reversable contraception 
  • General health counseling to promote healthy lifestyle habits including nutrition, eating disorders, and exercise
  • Dental services, including cleanings, exams, X-rays, and sealants 
  • Sports physicals
  • Routine lab work
  • Immunizations, including flu
  • Referrals to community health care and social support partners
