
Grant funding will connect patients to critical behavioral health services
[GLENWOOD SPRINGS, CO – May 6, 2025] Mountain Family Health Centers is pleased to receive a $220,000 grant award from The Colorado Health Foundation. The award will support Mountain Family’s behavioral healthcare integration utilizing a team-based care model, ensuring patients receive fully integrated medical, dental, and behavioral health care.
The grant funding comes at a critical time, as recent Medicaid redeterminations and rising health insurance premiums have forced many residents to go without health insurance coverage. Mountain Family Health Centers provides integrated health care on a sliding scale fee, where some appointment fees may be as low as $15.
Coloradans face significant barriers in obtaining mental and behavioral health support. Routine provider visits can cost well over a hundred dollars for uninsured individuals, and all patients – insured or not – are impacted by long waitlists due to increased need post-pandemic. The grant will help Mountain Family wrap behavioral health into a team-based care model for patients, ensuring they can access preventative interventions that will potentially save time and money. Physicians and behavioral health teams will work cooperatively to meet patient needs, exchanging critical information, engaging the patient in their own whole-person health plan, and reducing the likelihood of miscommunication. The model is proven to increase health outcomes and provider capacity, and reduce healthcare costs, wait times, and patient stress.
