Mountain Family Health Centers would like to welcome new subscribers to our monthly newsletter. Mountain Family provides medical, dental, and behavioral health services for more than 21,000 patients at our health centers in Edwards, Glenwood Springs, Rifle and Basalt, as well our school-based health center in Avon. Mountain Family provides additional dental services at our Mobile Dental Van in Glenwood Springs, Bea Underwood Elementary School and Grand Valley Family Learning Center in Parachute.
The Mountain Family Health Centers mission is to provide high quality, integrated primary medical, behavioral, and dental health care in the communities we serve, with special consideration for the medically underserved, regardless of ability to pay. We deeply believe in our motto: Our Family, Caring for Yours.
Our providers strive to provide the best care possible for our wonderful patients, which you can read about in our monthly Exemplary Care awards. To make an appointment please call 970-945-2840.
Mountain Family accepts all forms of health insurance, including Health First Colorado (Medicaid), Child Health Plan Plus (CHP+), Medicare, and private and commercial insurance. More than a third of our patients are enrolled with Medicaid, 5% have insurance through CHP+ and 4% have Medicare. This spring we’re offering insurance information at several events.
Uninsured patients are eligible for a sliding fee scale, based on income and each patient’s ability to pay. Approximately a third of our patients are uninsured.
Mountain Family is excited to be in the planning stages of expanding our Edwards health center. Mountain Family is fortunate to partner with a long list of community organizations in Eagle, Pitkin, Garfield, and Rio Blanco counties, as well as state and federal partners, making it possible to expand our patient numbers in Basalt and the Roaring Fork Valley.
