SBHC Summer

Mountain Family Health Centers has five School-Based Health Centers (SBHCs) located at Avon Elementary School, Basalt Middle School, Basalt High School, Glenwood Springs High School, and Roaring Fork High School in Carbondale. Patients under age 21 can be seen at any of our SBHCs year-round.

Our SBHC staff is dedicated to providing flexible services, even during the summer. They will continue to see patients for a variety of services. To schedule an appointment, please call 970-928-1701. If an enrollment form (ENG / SPA) is already completed, parents are not obligated to attend appointments with their child, providing a convenient option for busy schedules.

Sports physicals are now offered at the Eagle County and Roaring Fork School Districts. Sports physicals can be done alongside an annual well-child check-up and/or vaccinations. The cost for a sports physical appointment is $40, as they are generally not covered by private insurance or Medicaid for stand-alone appointments.   • Avon Elementary School: July 18 and July 25, 8am – 4pm (by appointment only)
  • Basalt High School: August 6 and August 13, 9am – 1pm (by appointment only)
  • Roaring Fork High School: August 1 and August 8, 8am – 12pm (by appointment and walk-in)
  • Glenwood Springs High School: August 7 and August 14, 8am – 12pm (by appointment and walk-in)
